The first phase of your new employee onboarding is Human Resources-related.
Right here we can knock out payroll forms, direct deposit information and the employee handbook. On your first day we’ll give you an office tour, set up your card key access and more!
We want to make sure you have everything you need - and we get everything we need - to get you off on the right foot at Lot Sixteen. Don’t hesitate to reach out to your onboarding coordinator(s) with any questions along the way.
Part I:
New Employee Information
When filling out the Employee Information Form above you will be asked to enter your banking account and routing numbers for paycheck direct deposits. In addition to this, it is helpful to have a letter from your bank that shows the routing number and account number for your bank account. Sometimes the routing number is different for direct deposit than for your everyday account activity. Please find out if your bank can provide this form, because it ensures the direct deposit information is correct and there won't be a delay in the delivery of your paycheck. Most banks can mail or email this letter, or they can direct you to a screen where you can find the information on your online account page.

Part II:
New Employee Payroll Forms
Each Lot Sixteen employee (full-time, part-time, and seasonal interns and fellows) must complete federal and state income tax withholding forms in order for our payroll services company to properly withhold the correct amount of income tax from your paycheck.

Most of the linked forms are online fill-in forms that can be filled out and downloaded. Then you can attach an electronic signature, or print, sign, scan and email the forms. (If you don't have a scanner, there is a great (free!) phone app called GeniusScan that makes it very easy to scan papers as a PDF and email them.)
Part III: The Employee Handbook
Review the Lot Sixteen Employee Handbook which can be accessed by clicking the button below. Please sign and return the acknowledgement page to your onboarding coordinator.
Please send your completed W-4, I-9 and State Tax forms along with your signed Employee Handbook acknowledgement page (four items total) to missy@lotsixteen.com
